Full-Time Case Manager (PATH)
Responsibilities
Provides outreach services to consumers with a history of mental illness in imminent risk of becoming homeless,as well as those who are currently homeless or residing in shelters.
Manages assigned caseload and continuously evaluates consumer’s service needs
Assists consumers in obtaining access to medical, social, rehabilitative, financial, housing and mental health services, in addition to any other qualifying services
Maintains accurate case records until consumer is transferred or discharged from program
Qualifications
BSW in social work, BA/BS in psychology, or a related field
Two years of previous experience in mental health and human services
Valid Driver’s License
