Full-Time Case Manager

Responsibilities

  • Provides case management and direct support to consumers both on site and in the community

  • Implements treatment plans, manages cases, and provides transportation and linkage.

  • Ensures the appropriate completion of all related documentation

  • Functions as an essential member of the team, assisting in shift coverage, participating program activities, and communicating with team members to cover all applicable duties

Qualifications

  • BSW in social work, BA/BS in psychology, or a related field

  • Two years of experience in mental health and human services

  • Valid Driver’s License

Pay

$40,000 - $43,000 annual salary

Shift

Sunday thru Thursday or Tuesday thru Saturday.
3 days shifts, 2 evening shifts.
