Maintains an individual caseload. Provides outreach services to the homeless population with mental illness and those at risk of becoming homeless. Includes assessing and linking consumers to services such as housing, financial benefits, psychiatric treatment and medical care. Maintains case records until consumer is transferred or discharged from program. Advocates for consumers with community gate keepers including state, county and local gov't., landlords, community groups, etc. Provides education and consultation to shelter staff as it relates to mental health.
Should have knowledge of community resources and be able to work in a team setting. Valid NJ driver's license required.
